Diagnostic Considerations

Other problems to be considered in the differential diagnosis include the following:

  • Sulfonylurea poisoning
  • Renta yam poisoning
  • Pregnancy with acute liver steatosis
  • Chronic valproic acid toxicity
  • Hypoglycemia due to pentamidine or quinine

While there is significant overlap in ackee poisoning and Reye syndrome symptoms (profuse vomiting, convulsions, seizures, coma) and laboratory findings (hypoglycemia, hyperammonemia, metabolic acidosis), ackee poisoning can be differentiated by increased glutaric and ethylmalonic acid levels in urine.
