Diagnostic Considerations

Autoimmune hepatitis

Acute hepatitis E presents with histologic and biochemical features of autoimmune hepatitis and, thus, acute HEV infection may be misclassified as de novo onset of autoimmune hepatitis—treatment with immunosuppressants in acutely HEV-infected patients may accelerate histologic progression. [33] Therefore, in patients with autoimmune hepatitis who will be treated with low doses of steroids with or without azathioprine, routine serologic testing of HEV infection should be strongly recommended in the initial work-up. Patients with autoimmune hepatitis who develop unexplained elevated liver enzyme levels should also be tested for HEV RNA. [33]
