Diagnostic Considerations

Autoimmune hepatitis

Acute hepatitis E presents with histologic and biochemical features of autoimmune hepatitis and, thus, acute HEV infection may be misclassified as de novo onset of autoimmune hepatitis—treatment with immunosuppressants in acutely HEV-infected patients may accelerate histologic progression. [33] Therefore, in patients with autoimmune hepatitis who will be treated with low doses of steroids with or without azathioprine, routine serologic testing of HEV infection is strongly recommended in the initial work-up. Patients with autoimmune hepatitis who develop unexplained elevated liver enzyme levels should also be tested for HEV RNA. [33]
