Diagnostic Considerations

Pseudohyperphosphatemia is most commonly due to paraproteinemia from the following:

  • Waldenström macroglobulinemia
  • Multiple myeloma
  • Monoclonal gammopathy of unknown significance

Other differentials to be considered include the following:

  • Vitamin D intoxication
  • Oral saline laxative (Phospho-soda) abuse
  • Pseudohyperphosphatemia
  • Pseudohypoparathyroidism
  • Rhabdomyolysis
