Guidelines Summary

The American College of Radiology ACR Appropriateness Criteria® for imaging an indeterminate renal mass include the following recommendations for initial imaging [46] :

  • No contraindication to contrast CT or MRI: Abdominal CT without and with IV contrast, abdominal MRI without and with IV contrast, or  abdominal ultrasound with IV contrast are equivalent modalities and usually appropriate.
  • Both iodinated CT or gadolinium-based MRI are contraindicated: Abdominal ultrasound with IV contrast, retroperitoneal kidney ultrasound, or abdominal MRI without IV contrast are equivalent modalities and usually appropriate.
  • Iodinated CT is contraindicated: Abdominal ultrasound with IV contrast, or abdominal MRI with or without IV contrast are equivalent modalities and usually appropriate.
